Residents if Quezon City, Las Piñas, and Quirino Province can register on eZConsult for COVID-19 vaccination. Your LGU determines all the guidelines including eligibility and scheduling. Soon, we will also include other LGUs.

Being eligible means you can get vaccinated. Your LGU determines your eligibility by checking if you're a resident or employee in the area and if you fit the priority category for getting COVID-19 vaccines.
It takes between a few days to a few weeks after submitting your CEIR form.
We will notify you via email, SMS, and in-app once you're eligible and can schedule a vaccine appointment. If you didn't receive a notification, please contact your LGU.
No. After the approval of your CEIR form, you will need to schedule an appointment on the eZConsult website or app. The available schedules are given and controlled by your LGU.

Your LGU may not have open vaccination sites or you haven't provided your correct address. Only ELIGIBLE residents and employees in Quezon City, Las Piñas, and Quirino Province can book through eZConsult. If you're from another area, please contact your LGU directly.
It's a unique identifier assigned by certain LGUs to each citizen. In Quezon City, this is called the QC ID. Other LGUs may have similar ID systems. Please indicate your Category ID such as your QC ID to complete your CIER form submission.
For Quezon City residents with registered QC IDs, enter "QC1" then the valid ID number (example: OC1-xxxxx). For Las Piñas residents, put "LP1". For Quirino Province residents, put "QU1".
Please ensure you have a stable internet connection and use the latest version of the eZConsult app. Your phone must also be running on at least iOS version 10 or Android version 8. If not, you may use our website.
Kindly retry the process. Telecoms are responsible for sending the OTPs. Our data shows that only 70% of OTPs are sent back by telecom providers. If you need further help, don't hesitate to contact eZConsult support.
